Outcomes of Endoscopic Resection for Colorectal Polyps With High-Grade Dysplasia or Intramucosal Cancer

Background and Aims Little is known about the outcomes of endoscopic resection (ER) for patients with colorectal adenomas (CRAs) high-grade dysplasia (HGD) or intramucosal cancer (IMCA). This study aimed to estimate rate local/malignant recurrence, identify predictive factors local recurrence (LR), evaluate treatment after ER HGD/IMCA. Methods A retrospective review was performed who underwent HGD/IMCA in 2 academic medical centers. Risk LR were determined by Cox regression analysis. Results Overall, 188 lesions included; 61 removed en-bloc (e-ER), whereas 127 a piecemeal (p-ER). The mean lesion size 20.3 mm. Of 125 follow-up, adenoma occurred 31 (23%), malignant (1.6%) at median follow-up 16 months. ≥ 4 cm p-ER have greatest hazard ratio (HR = 21.5; 95% CI 2.5-180.5; P 0.005) LR, compared < e-ER. Surgery 3.2% complete ER, all p-ER. had 22.6% (7/31 patients) recurrent despite repeat attempts 1.9 ± 0.79 procedures from index ER. Conclusion Our demonstrates high (23%) CRAs 1.6%, especially Thus, e-ER should be preferred these whenever technically feasible.
